Routine Referrals

Please note ISP’s are available in Hampshire and these are not listed below.

Portsmouth & SE Hampshire

Either via the GP or to Practice Plus Group (St Mary’s) directly (if applicable). Glaucoma and Cataract patients should be referred to Practice Plus Group (St Mary’s) NHS Treatment Centre.

Practice Plus St Mary’s NHS Treatment Centre:

Optometrists can send a GOS 18 referral to Practice Plus St Mary’s Ophthalmology Service either via email or post. Please read this PPG Referral Guide for further information. Remember to state on the referral letter for cataract if the patient has had a laser refractive treatment in the past.

Southampton

Send to the patient’s GP – allow at least four weeks for a patient to receive an appointment letter before checking on the status of their referral, should this be necessary – call patient services centre 023 81 205009
Please note that the ‘patient services centre’ deals with eye appointments for Lymington, Hythe and Romsey Hospitals as well as UHS (also known as ‘the General’).

Refer to Solent Medical Services (SMS) Eye Clinic for:

  • Suspected Glaucoma/Raised Intra Ocular Pressure
  • Cataracts
  • Dry Eyes
  • Blepharitis
  • Flashing lights or floaters lasting for 5 days or more

Basingstoke and Mid Hampshire

Refer to the patient’s GP.

Frimley Park (including Heatherwood and Wrexham Hospitals)

Routine cataract referrals can be sent directly to Frimley Trust via email (fhft.ophthalmology.referrals@nhs.net).

Direct Access Cataract Referral Form – .docx Format – .pdf Format

Routine non-cataract referrals should continue to be sent to the patient’s GP.
