UHB accept cataract and YAG referrals for the following sites:

Birmingham Heartlands Hospital, Bordesley Green E, Birmingham B9 5SS
Solihull Hospital, Lode Ln, Solihull B91 2JL
Queen Elizabeth Hospital Birmingham (QE), Mindelsohn Way, Birmingham B15 2WB
Good Hope Hospital, Rectory Road Sutton Coldfield West Midlands B75 7RR

The best way to make a cataract referral is directly via the Opera platform, more information here.

Otherwise you can refer via NHSmail:

uhb-tr.appointments-centre@uhb.nhs.uk for Solihull/Heartlands/Good Hope

Or via GP on GOS 18. If you use this route make sure you are clear why you cannot refer via Opera and that the GP must action the referral.

Make sure to indicate clearly which site the patient wishes to be seen at. They should also be made aware that in certain circumstances they may be seen at an alternate site in the group.

YAG referrals can also me made to the above email address. Again if you have no choice but to refer via the GP on a GOS 18 you must make clear that the GP must action the referral.
