Tell us your problems!
No, we’re not starting a counselling service but we do want you to tell us about your concerns, poor experiences and difficulties relating to the current referral pathways and lack of services in Hertfordshire.

We are hoping to collect as much data as possible to strengthen our drive to get services commissioned, to help you, and ultimately our patients, such as:

  • Situations where a Minor Eye Conditions Service (MECS or CUES) would have benefited Px care or their experience
  • Problems caused by the lack of Out of Hours Emergency Cover in East and North Herts (difficulty in referring, potential or actual impact on care outcome, px safety etc)
  • Being asked to do, or doing, extra procedures or providing extra information at the request of a Trust, Provider or Consultant (such as providing scans, field tests etc) that would have been funded if a commissioned pathway was in place.
  • Pxs advising us that they did not get their preferred provider or were not offered choice after referral to GP (East and North Herts) or CHEC (South and West).  
  • Problems caused by having to email or phone before sending an urgent referral
  • Any other situations where px care has or could have been compromised, or a referral process was harder than it should have been if a service or pathway was an option

We have created a very simple form for you to complete HERE. You can complete multiple forms if you have had several issues.

We’re sorry we may not be able to comment or follow up on individual issues but really appreciate your support with this.

You can include anything current or from the last few months, and please can you keep the form bookmarked to tell us about future issues. Please don’t include patient identifiable data (of course) and it is up to you if you want to include your details, although it strengthens our case with the ICB if you do. Everything on the form is optional.
