We have 3 items for you today:
- Tell us more of your problems!
Thanks to everyone who has completed our survey form regarding issues around referrals. We would really like more examples, please, especially specific instances where Px care or even safety has been at risk or suffered.
We have created a very simple form for you to complete HERE. You can complete multiple forms if you have had several issues.
You can include anything current or from the last few months, and please can you keep the form bookmarked to tell us about future issues. It can also be found on our Website, HERE.
We’re sorry we may not be able to comment or follow up on individual issues but really appreciate your support with this.
Please don’t include patient identifiable data (of course) and it is up to you if you want to include your details, although it strengthens our case with the ICB if you do. Everything on the form is optional. Any queries, email us at admin@HertsLOC.org.uk
Thank you for your time.
