Routine referral guidelines – Barking & Dagenham, Havering & Redbridge

All referrals are done via Evolutio.  These can be done in a number of ways:

• GOS 18 (or equivalent) faxed to Evolutio on 0333 240 7729

• GOS 18 (or equivalent) emailed to lchevolutio@nhs.net
  • Must be emailed from an NHS email address
