Glaucoma Services Patient Discharges from NuTH (Oct 2024)

For those Optometry practices registered to accept stable glaucoma and OHT monitoring.
NuTH have now begun to discharge patients from into this service, so please ensure that you regularly check your OPERA platform.
Patients will appear in the “action required” section on OPERA.
Please check the documents that have been attached to the patient record and ensure that you have all the required clinical detail before accepting the patient. If you have any questions regarding the services, then please contact Angela or Naomi.
Please do not reject patients back to NuTH without discussing the issues with Angela or Naomi.
Patients have received communications from NuTH that they are being discharged into this service, but they may require some support in understanding why it is important that they attend your practice for these assessments and the difference between this appointment and a sight test.
Patients can continue to attend their usual practice for sight tests.
If a patient wants to change their provider practice please ascertain where they would like to attend, ensure that this is a provider practice for Glaucoma services and contact the PES team via hello@referral.support to advise of the changes required. (This is all documented within the guide attached)
