Information required from practices wishing to provide the Children’s Service

The following documents are designed for those practices interested in providing the Children’s Service which is commissioned by Primary Eyecare North East Limited (PENE) from participating CCGs in the Northumberland, Tyne & Wear LOC area.

New contracting arrangements came into force in January 2016. Before a practice can provide this service under these new contracting arrangements a contract must be in place between the practice and PENE.

The ‘Practice Registration’ and ‘Expression of Interest’ forms collect the information which PENE requires to prepare the contractual documentation, set up your practice to provide the service and ensure payment is made to your practice for the services provided.
